As Meet Host (Meet Director, Representative), I am agreeing to abide by all the rules set forth below and by USA Swimming for conducting either a sanctioned or approved meet. - You will sign at the bottom.

As a condition of obtaining such a sanction, I and the above organization, which I represent, agree to abide and govern this event under the rules and regulations of USA Swimming, Inc., and Colorado Swimming, Inc., and all other terms and conditions upon which this sanction may be granted. These terms specifically include all local rules and regulations and those set forth in Article 202 of the current edition of USA Rules and Regulations, specific reference to Article 202.2.7

"In granting this sanction it is understood and agreed that USA Swimming shall be free and held harmless from any liabilities or claims for damages arising by reason of injuries to anyone during the conduct of the event."

As Meet Director/Representative, I have agreed to work closely with the meet referee and entry chair before the meet, running a time line as the entries are received. Once the four hour rule is reached, entries will be closed. If our meet has events that require timers to move to the other end of the pool, we will figure that into our time lines. Intervals will not be set below 15 seconds, unless pre-approved by Technical Planning. I will also send to the CSI office a meet registration check after the entry deadline has passed.

I will ensure that the meet finacial report is submitted within 45 days of the conclusion of the meet. FINANCIAL REPORT FORM

Fees (will be charged to your account after the meet has been conducted):

Under 150 swimmers = $25 fee
151-400 swimmers = $100 fee
401-900 swimmers = $150 fee
901+ swimmers = $200 fee

League fees = $200 for up to 8 meets, $25 for any additional after that. All sanctioning requirements apply to league meets.

Required Splash Fees:

All meets except league meets will be billed $1.20 per event swum after the meet has been processed.

Sanction Number will be issued after meet referee sign off has been received.
