By signing, I AGREE to support my son or daughter in this Ministry of St. Bede-St. Denis as an altar server on the days and times indicated in the Schedule, for Masses, formation days and recreation days. I further AGREE to bring my son or daughter at least 15 minutes before each Mass, and to make sure that he or she is dressed appropriately. I also understand that if my child is late and/or is not dressed properly, he or she may not be allowed to serve at that Mass.*
