Proxy Form Rules
- The Member themselves must complete and sign this Proxy Form, regardless of age. Minor members, must also have their parent or guardian sign the form.
- If multiple Proxy Forms are received from the same Member, all proxies (original and duplicates) for that Member will be declared void. The member or proxy holder will not be notified of any duplications.
- Proxy Forms must be completed correctly and in their entirety; incomplete forms will be considered void.
- The deadline for Proxy Form submission is November 6 at midnight. No forms or changes will be accepted after this date and time.
