This form will open on Wednesday, Oct. 1 at 12 p.m. and close on Sunday, Oct. 5, at 11:59 p.m. EST
This form is for families facing economic hardship during this Christmas season in the Piedmont Triad area in North Carolina. This is for families with children under the age of 17.

Please complete the form to be considered for adoption. Sign up is not a guarantee of placement.

If you are selected, a member from I Am A Queen will contact you via phone.

If you are applying for yourself and a friend, it is not a guarantee the two of you will be considered together.

Please note: First priority will go to NEW families. If you were on the list previously, it is not a guarantee for selection.
