If you are interested in receiving a Thanksgiving Food Basket, please fill out the information below and return it to us no later than November 1, 2025. Your information will be shared with organizations and individuals interested in providing the baskets. You will be contacted by them directly and they will give you instructions on delivery or pick-up. Please be sure to answer your phone and check your email for correspondence. We cannot guarantee you receive a basket if you cannot be reached. Basket content will be determined by the persons or organizations themselves. We cannot guarantee any specific item or amount that you will receive.
