This Jotform is to be completed for members who are to be appointed NEW Endorsed Delegates for Shallow Water Rescue Induction.

Endorsed Delegates for Shallow Water Rescue Induction must be:

• Proposed by club Chief Training Officer (CTO) to conduct inductions for club members for Shallow Water Rescue only.
• Re-endorsed annually and meet the same criteria as listed below each season.

Instructions:

• The CTO is to list the proposed delegate’s full name and DOB below.

Pre-requisites for  Endorsed Delegate Shallow Water Rescue

• Hold current Bronze Medallion, OR
• Hold current Surf Rescue Certificate
• Hold QLD Shallow water Rescue Induction Award

 Admin Only

• The CTO will be informed once members have been updated in Surfguard and permissions added to aXcellerate.
