Repair & Refund Policy

Free Repairs: Smith + Chain Co. offers free repairs as long as the customer still has the jewelry in their possession.

Lost Jewelry: Jewelry will not be replaced by Smith + Chain Co. if lost.

No Refunds: All sales are final. Refunds are not offered after purchase.

By signing below, I acknowledge and accept this policy.

Acknowledgment of Risks

I am aware and understand that permanent jewelry requires the procedure of welding jewelry, and that such procedure involves potential risks, including but not limited to:

Risk of serious injury, pain, burns, or scarring.

Risk of infection, allergic reaction, or skin irritation.

Risk of jewelry becoming loose, breaking, or falling off.

Risk of exposure to COVID-19 or other infectious diseases while on the premises or during the procedure.

I acknowledge these risks and voluntarily consent to undergo this procedure with full knowledge of the potential dangers involved.

Health & Safety Statement

By signing below, I confirm that I:

Am in good health and proper physical condition.

Do not have medical conditions that would impair my ability to undergo this procedure.

Am not experiencing symptoms of any contagious illness (including COVID-19).

Will comply with all instructions, safety precautions, and aftercare guidelines provided by Smith + Chain Co.

Emergency Medical Consent

If I am injured or require medical attention during my appointment, I consent to receive necessary medical treatment. I understand I am solely responsible for all related costs.

Liability Waiver & Release

In consideration of participating in the procedure to obtain permanent jewelry, I hereby waive, release, and discharge Smith + Chain Co., its owners, employees, contractors, and affiliates from any and all claims, liabilities, or causes of action that may arise out of my participation, whether caused by negligence or otherwise.

This release does not extend to claims for gross negligence, willful misconduct, or any liability not permitted to be released under Washington or Oregon law.

Media Release

Smith + Chain Co. occasionally photographs or records appointments for promotional use (website, social media, marketing).
