Thank you for your interest in joining Navigating Change! This program is designed to support Transition-Age Youth (ages 16–22) and their families. Navigating Change is a hybrid program—while most activities will take place virtually, there will be occasional in-person events that you’ll be invited to attend.Each family will be paired with a Family Transition Specialist and a Youth Mentor who will provide ongoing support and connect you with local resources and services.
