This is the approval form that will be used by a school administrator to receive permission from the GACS office to offer a Continuing Education Training that can be used as CEU's toward the GACS Professional Educator Certification Program.

This form is specefically for training sessions that are not directly GACS sponsored, yet the administrator believes would be pertienent for their staff for the purpose of professioanl development.

This would be the name of the class, book title, etc.
What organization is providing this training?
If its being provided in house, put the name of your school.
What professional practices are to be developed or enhanced by the training activity?*
What teaching strategies will be used? Workshops? Lecture? Book Study? Group Work? Etc.?*
What kind of assessments will you use to ensure that the participants have met the goals and objectives of the professional training?*
How many CEU units are you seeking for this training?*
1 CEU = 10 contact hours of training

This question is because we can offer partial CEU's if the training is not a full 10 hours.
