I understand the risks associated with participation in discovery sessions and voluntarily assume full responsibility for any injuries and conditions that may arise. I am aware that OTS Academy is not liable for any injuries that may occur during participation in any of the sessions. As the parent/guardian of the participant, I have read and give my consent for participation.

We accept e-transfer to info.otsacademy@gmail.com or payment by credit card. Note: To pay by credit card, please reach out to otsacademy.admn@gmail.com There will be a 3.5% administrstion fee added to all credit card payments.

All registration fees are non-refundable.

Once payment is received, no refunds will be issued for any reason, including but not limited to illness, injury, scheduling conflicts, or withdrawal from the skills clinic.

In the event that the skills clinic is canceled by OTS Academy, participants will be offered either a rescheduled date or a credit towards a future clinic.
