2026 Winter Meeting Registration Form

Thank you for registering for the upcoming FAP Winter Meeting.

Some information to assist you:

1. The biennium for re-licensure ends on February 28, 2026. This meeting has tentative 2.5 clinical/scientific CEUs (Friday) and 3 practice management CEUs (Saturday).

2. Make sure to register your dental team members for the Saturday lecture early, there is limited space in the lecture room.

3. For dental team members - lunch is on their own during the FAP business meeting luncheon on Saturday.

4. Please notify the FAP office of any changes to your registration as soon as possible by emailing, fap@tampabay.rr.com.

5. At this time, there are no corporate forums scheduled for Saturday morning. An email will be sent if this changes.

6. Cancellation Policy - For a full refund, you must cancel your registration by January 20, 2026.
