In consideration of being allowed to participate in the player tryouts organized by STORM 9U FLAG FOOTBALL , I, the undersigned, on behalf of myself, my heirs, executors, administrators, and assigns, hereby acknowledge and agree to the following:

Assumption of Risk: I understand that participation in player tryouts involves inherent risks, including but not limited to physical injury, illness, property damage, and the risk of contracting contagious diseases. I voluntarily and knowingly assume all such risks associated with my participation.
Release and Waiver: I, for myself and my heirs, hereby release, discharge, and hold harmless STORM 9U FLAG FOOTBALL, its officers, directors, coaches, volunteers, and all associated personnel, from any and all claims, demands, actions, or causes of action, whether for personal injury, property damage, or otherwise, arising out of or in connection with my participation in the player tryouts, including any actions or negligence on the part of [Organization Name] and its personnel.
Medical Treatment Authorization: I authorize STORM 9U FLAG FOOTBALL and its representatives to seek and obtain any necessary medical treatment or attention in the event of an injury, illness, or medical emergency during the tryout. I understand that BAA STORM FLAG FOOTBALL will make reasonable efforts to contact me or my emergency contact before seeking medical treatment, but they may proceed with necessary medical care if immediate attention is required.
Compliance with Rules and Guidelines: I agree to comply with all rules, guidelines, and instructions provided by STORM 9U FLAG FOOTBALL during the player tryouts.
Parent/Guardian Consent: If the participant is a minor, the parent or legal guardian must consent to this waiver and release of liability on their behalf.
I have read and understand this Waiver and Release of Liability, and I voluntarily sign it as my own free act and deed. I am aware that by signing this document, I am waiving certain legal rights that I or my heirs, executors, administrators, and assigns may have against STORM 9U FLAG FOOTBALL.
