Updating Your Agency's Information?
Please use this form to notify us about any updates to your agency's information including changes to your name, website, main address, or contact information. This is also the correct form to use to notify us if you need to provide new contact information for the staff members in your organization who are serving as your Executive/Director/CEO, HMIS Security Office, or HMIS Lead.
Requesting an Agency Deactivation?
Before we can close down an agency in HMIS, we must first ensure that all of your clients have been exited from your programs and then close those programs down. Only when all activity in HMIS has been ended and resolved can we move forward with the deactivation of your agency. If this is why you are filling out this form, please make sure you have submitted a separate Update HMIS Program Request form for each program you have.
