J’S TOP DOLLAR PROFESSIONAL TAX SERVICE
Client Engagement & Acknowledgment Agreement
1. Accuracy of Information
I acknowledge that all information provided to J’S TOP DOLLAR PROFESSIONAL TAX SERVICE is true, correct, and complete to the best of my knowledge. I understand that I am required to provide all supporting documentation necessary to substantiate the information reported on my tax return.
I further understand that knowingly providing false, misleading, or incomplete information for the purpose of preparing a tax return is a violation of federal law and may result in civil and/or criminal penalties, including fines, restitution, and imprisonment.
I release J’S TOP DOLLAR PROFESSIONAL TAX SERVICE and its preparers from any liability or penalties arising from errors or omissions caused by inaccurate or incomplete information provided by me.
2. Authorization to Prepare and File
By signing below, I authorize J’S TOP DOLLAR PROFESSIONAL TAX SERVICE to prepare my federal and/or state income tax return(s) based on the information I have provided.
I understand that the return cannot be filed without my signature, and I acknowledge that tax preparation fees are earned upon completion of the return, regardless of whether I sign or file it. Refusal or delay in signing does not relieve me of responsibility to pay all fees due.
3. Responsibility for Tax Preparation Fees
I understand that fees for tax preparation services are due at the time services are rendered.
If I choose to have fees deducted from my refund and my refund is reduced, delayed, or offset by any government agency (including IRS, child support, student loans, or other debts), I remain fully responsible for paying the full preparation fee directly to J’S TOP DOLLAR PROFESSIONAL TAX SERVICE.
If fees are not paid within 30 days of invoicing, I agree that J’S TOP DOLLAR PROFESSIONAL TAX SERVICE may pursue collection remedies, including referral to a collection agency, filing a lawsuit, obtaining judgment, and seeking post-judgment remedies (including wage garnishment, bank levies, or liens) to the extent permitted by law. I also agree to pay any court costs, collection agency fees, reasonable attorneys’ fees, and interest on the unpaid balance.
Prior to initiating legal proceedings, I will receive written notice of the delinquent balance and a 15-day opportunity to pay or submit a written dispute. Venue for any legal action shall be in the state/county where J’S TOP DOLLAR PROFESSIONAL TAX SERVICE maintains its principal place of business.
4. Sitting / Preparation Cancellation Fee
Client acknowledges that if they choose to cancel tax preparation services after work has begun, or decide not to sign the return after J’S TOP DOLLAR PROFESSIONAL TAX SERVICE has prepared it, a sitting/preparation fee of $100 will be charged.
This fee covers the time, effort, and resources spent preparing the return up to the point of cancellation. The sitting fee is non-refundable and due immediately, regardless of whether the return is ultimately filed.
Revocation of Authorization:
Client may revoke this authorization in writing prior to completion of services; however, revocation may delay preparation or filing of the tax return and does not relieve Client of responsibility for fees earned.
Acknowledgment:
By signing below, Client acknowledges that they have read, understood, and agreed to this authorization, and that this authorization constitutes written consent to charge the card for fees as outlined above.
6. IRS Communication (Optional Representation)
In some cases, J’S TOP DOLLAR PROFESSIONAL TAX SERVICE may need to communicate directly with the IRS regarding my tax return or account. To do so, the IRS requires a signed Power of Attorney (Form 2848) or Tax Information Authorization (Form 8821). These forms do not authorize control of funds or personal accounts; they only allow communication with the IRS on my behalf.
7. Engagement Scope & Client Acknowledgment
Client understands that the preparation of the return does not include audit representation unless explicitly agreed upon.
Client acknowledges that preparation fees are earned upon completion, even if the return is refused, unsigned, or not filed.
Client agrees to provide all necessary documents and respond to preparer inquiries in a timely manner.
