1. Acknowledgement of Risks 

I understand tattooing carries risks including pain, bleeding, infection, allergic reactions, permanent scarring, dissatisfaction, and possible need for medical treatment. I voluntarily assume all risks.

 

2. Eligibility & Age Verification

I certify I am 18+ OR, if 16–17, my parent/legal guardian has signed below.

I understand falsifying my age or identity is fraudulent and release The Tattoo Nation from any resulting liability.

 

3. Health Disclosure

I affirm I am not under the influence of drugs or alcohol.

I am not pregnant or breastfeeding. I have no medical condition that interferes with tattoo healing, or I have disclosed it in writing. I accept full responsibility for any undisclosed risks.

 

4. Payment & Refund Policy 

I acknowledge that all payments made for tattoo services are final and non-refundable, regardless of outcome, dissatisfaction, or healing complications.

 

5. Release of Liability 

I irrevocably release The Tattoo Nation, its owners, artists, and employees from all claims relating to the tattoo procedure,

including dissatisfaction, infection, scarring, allergic reaction, or long-term changes. This release binds me and my heirs.

 

6. Aftercare Responsibility 

I have received Tattoo Nation’s Aftercare Instructions.

I understand complications may occur if I fail to follow them, and I accept full responsibility.

 

7. Parent/Guardian Consent (16–17 only) 

I, the parent/legal guardian, consent to tattooing my child. I release The Tattoo Nation from liability, accept all risks, and agree this release binds me, my child, and heirs.

 

8. Emergency Medical Consent 

In the event of a medical emergency, I consent to emergency medical treatment and release The Tattoo Nation from any liability relating to such care.

 

9. Legal Effect & Enforceability 

This waiver is binding in perpetuity. Disputes will be resolved by binding arbitration in Maryland. If any clause is invalid, the remainder remains fully enforceable.
