Indemnity and Consent Form
I, the undersigned,
hereby acknowledge and agree to the following:
Participation at Own Risk
I understand and accept that rugby is a sport that involves physical activity and may carry the risk of injury. I voluntarily participate in all activities, matches, practices, tournaments, and events organized by Thistle Ruby Football Club at my own risk.
Indemnity
I hereby indemnify and hold harmless Thistle Rugby Football Club, its officials, coaches, sponsors, and affiliates from any claims, damages, medical costs, or legal actions arising out of injury, loss, or damage to my person or property sustained while participating in club activities.
Medical Consent
In the event of an emergency, I authorize the club officials to secure medical treatment on my behalf, and I accept responsibility for any costs incurred.
Code of Conduct
I agree to abide by the club’s rules, regulations, and code of conduct, and to show respect to teammates, opponents, officials, and the game of rugby at all times.
Personal Information (POPIA Compliance)
I consent to the collection and use of my personal information by Thistle Rugby Football Club solely for administrative, communication, and rugby-related purposes, in line with the Protection of Personal Information Act (POPIA).
Declaration
I confirm that I have read and understood the contents of this indemnity form, and I voluntarily agree to the terms stated herein.
