I am aware that the congregation of horses at an event provides an opportunity for the spread of infectiousdiseases.

I declare that the horse(s) named above has/have been in good health and eating normally if this changes with close proximity of the clinic i will endevor to notify Indian Ocean Reiners and western performance assc.

I give my authorization for the designated Animal Health Officer to arrange for a veterinary examination of the horse(s) named above if it/ they show signs of a serious infectious disease or excessive distress at the event. I agree to pay any veterinary fees incurred as a result of this veterinary examination.

Conditions and Responsibilities
• Deposits of $200 or 50% (whatever is greater) must be paid at the time you nominate for the clinic. Unless prior arranged with committee of Indian Ocean Reining and Western Performance Assoc.

***If funds are not received your position will be delegated to the next in line.

• Full payment is due 1 week prior to clinic date.

• Deposits, Half Payment and Full Payment are non refundable unless the clinic is cancelled by Indian Ocean Reining and Western Performance Assoc.

• Horses must wear a bridle with a bit or a bosal for riding no halters or bitless bridles are allowed.

• All tack must be in safe and usable condition -Indian Ocean Reining and Western Performance Assoc has the right to prevent riding if tack is deemed unsafe. Once rectified or changed you may ride.

• Riders must be 10 years of age or older

• If a rider cannot attend the clinic, they can sell their position to another rider. However Indian Ocean Reining and Western Performance Assoc.
must be notified of the change and forms must be completed by the new rider.

• If a rider is selling their position. Payment is to be agreed to between the two riders. Indian Ocean Reining and Western Performance Assoc will not be responsible for transfers of funds between two riders.

• In the even of horse inury a vet certificate must be supplied and discussion with Indian Ocean Reining and Western Performance Assoc to see if refund maybe obtianed.
