Consent of Custody

I hereby voluntarily relinquish custody of my pet to Angel Oaks and authorize Angel Oaks and its authorized staff or representatives to take possession of my pet and to provide aftercare services, including cremation, in accordance with my wishes, effective immediately upon signing. I confirm that the information I have provided is accurate and complete, that the nature and purpose of the services have been explained to me, that fees have been disclosed, and that I accept full financial responsibility. I release and hold harmless Angel Oaks and its agents from any liability arising from the care, handling, and cremation of my pet.
