I CERTIFY that I am the actual owner of the dog, or that I am the duly authorized agent of the actual owner of the dog entered hereon.

In consideration of the acceptance of this entry, I (we) have read, understand and will abide by the Official Rules and Regulations of the Agility Association of Canada, Inc. in effect at the time of this entry, and by any additional rules and regulations appearing in the Agility Trial Schedule for this event. I (we) certify and represent that the dog entered is not a hazard to persons or other dogs. This entry is submitted for acceptance on the foregoing representation and General Agreement/Disclaimer. I (we) agree to conduct myself (ourselves) in accordance with such Official Rules and Regulations (including all provisions applying to discipline) and to abide by any decisions made in accordance with them.

In consideration of the acceptance of this entry, the holding of the event and/or the opportunity to have this dog participate or be judged, I/We agree as follows:

1. while at or in proximity to this event, to freely assume all risk of loss (including death) or damage or injury, collectively called "Loss", to myself, this dog or any dog under my control, and to members of my family and my guests, and to our property;
2. to release and discharge and to hold harmless and indemnify the organizing committee, the club and its directors, officers and members, the sanctioning body for the event (if any), sponsors of the event (if any), and the owners of the premises grounds upon which the event is held and their respective employees and agents, collectively called the "Releasees", of and from any and all claims, and from all liability of any nature, for all Loss which I or this dog or any dog under my control or members of my family or my guests or our property may suffer for any reason or cause whatsoever, including specifically, but without limitation, all Loss resulting from the action of inaction of any dog, including this dog, or its owner or handler, or by the Releasees, or any of them, whether through negligence or otherwise. I/We expressly assume the risk and liability for all such Loss while attending or in proximity to this event and waive any and all claims relating thereto;
3. to hold harmless and indemnify the Releasees, and each of them of and from any and all claims by third parties, and from all liability of any nature, for all Loss, in relation to claims against me or through me, or against or through any member of my family or any other person accompanying me or attending as my guest or in my place, or arising as a result of any action or inaction by me or by this dog, or its owner or handler, or by any member of my family or any other person accompanying me or attending as my guest or in my place, or by the Releasees or any of them in relation thereto, whether through negligence or otherwise. I/We expressly assume all responsibility and liability for all such claims; and
4. that all indemnities herein shall be full and complete indemnities, including legal costs on a solicitor and client basis.

I/We, the undersigned, agree that the club holding this event has the right to refuse this entry for any reasons which the organizing committee deems sufficient. I/We further agree to volunteer to provide assistance to the trial in the form specified by the organizing committee.

I/WE ACKNOWLEDGE that I/We have been given the opportunity to read this form before signing it, that I/We have in fact done so, that I/We fully understand the contents of this form and that it restricts my legal rights, and that I am signing this form voluntarily and of my own free will.

I/We also acknowledge that in the circumstances of COVID-19 still being present, there is a risk of my/us contracting this disease as a result of associating with participants, judges, spectators and others during the course of participating in any event at this facility this year. My/Our risk includes illness (short & long term) and in severe cases, death. I/We are also at risk of communicating this disease (however or wherever contracted by me) to others who may then experience greater or lesser illness and in severe cases, death.

In the event of a discrepancy between directives of the owners and/or hosts decreed during general briefing and any current Public Health order or guidelines, the Public Health directives shall prevail.

If I/We become ill or am concerned that I/We may have been exposed to Coronavirus, I/We agree to notify the appropriate Health Authority, and to inform the owners of the premises, hosts and any other individuals that I/We may have been in contact with, promptly and without delay.
