Yoga Class Liability Waiver & Acknowledgment
I acknowledge that participation in yoga involves physical activity and carries the potential for injury. I voluntarily assume all risks associated with participation in this class, including but not limited to injury, illness, or accident. I release and hold harmless the Sherburne Sports Complex, its instructors (including Jackie Craine), owners, employees, and affiliates from any and all liability, claims, or demands arising from my participation. I certify that I am physically able to participate and will consult a physician if I have any medical concerns.
Payment Information: Payment is $15 per participant, due upon arrival at the class. We accept cash or check only, payable to Sherburne Sports Complex. Please arrive a few minutes early to check in and submit payment before class begins.
