Sherburne Sports Complex — Youth Training Session Liability Waiver & Photo Release

In consideration of my/our child being allowed to participate in any way in the Sherburne Sports Complex Youth Training Sessions and related events or activities, I acknowledge, appreciate, and agree that:

• The risk of injury from athletic training activities is significant, including the potential for serious injury, permanent disability, or death. While rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist.

• I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of Sherburne Sports Complex, its instructors, coaches, volunteers, or representatives, and I accept full responsibility for my child’s participation.

• I agree to comply with all stated and customary terms, safety rules, and facility policies during participation. If I observe an unusual or significant hazard, I will immediately bring it to the attention of a staff member or coach.

• If my child leaves a session for any medical reason, he or she may not return without written clearance from a licensed physician. Coaches and staff reserve the right to require medical clearance at any time before a participant resumes training.

• I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Sherburne Sports Complex, its owners, instructors, officials, agents, volunteers, sponsors, and property owners or lessors from any and all injury, disability, death, or loss or damage to person or property—whether arising from the negligence of those released or otherwise—to the fullest extent permitted by law.

• I also consent, without further consideration or compensation, to the use of photos, audio, or video recordings of my child or family members taken during sessions or events for documentation, promotional materials, or web and social media content. I release Sherburne Sports Complex and its representatives from any liability arising from such use.

I have read this release of liability and assumption of risk agreement. I fully understand its terms. I understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
