CAST AGREEMENT
By submitting the registration form, I acknowledge that lack of attendance or repeated disruptive behavior is sufficient grounds for LSA to revoke my or my minor child’s privilege of participation in LSA’s Nutcracker on Ice. LSA will not refund any registration or costume fees in the event my or my minor child’s privilege to participate in this program is revoked. I agree that absences will only be excused at the sole discretion of the Artistic Directors. Excused absences include academic testing (such as high school placement, SAT or ACT, or similar tests), another skating event, and family emergencies. Any absences must be approved by the Artistic Director in advance. If I or my child are ill or injured, I/we will make every effort to be present to watch rehearsal and/or to catch up on any choreography my group’s coach deems necessary.
VOLUNTEER AGREEMENT
By submitting the registration form, I acknowledge that I, or at least one member of my family, am expected to volunteer for at least two performances and to help serve on necessary committee(s). I recognize that my active participation is very important to the success and future of the show and of LSA and that I may be unable to watch the performance(s) that I work. I also understand that my refusal to volunteer is grounds for my or my child’s dismissal from the cast and that no refund will be issued. I AGREE THAT I HAVE READ THIS AGREEMENT, HAVE HAD AN OPPORTUNITY TO ASK ANY QUESTIONS ABOUT IT THAT I MAY HAVE, AND THAT CHECKING THE BOX ON THE REGISTRATION FORM INDICATES MY AGREEMENT TO ALL OF ITS TERMS.
