RENEWERS!  USSPWD Club welcomes you back for another fun filled year with our  Portuguese Water Dogs and our Family of Members.  Please type in the email address that you registered with last year (all letters in lower case)and then click the "AUTO COMPLETE FIELDS" button below your email address and your current membership profile will populate on the form.   Please update any fields that need updating, make your membership type selection for the upcoming year at the bottom of the page and hit the SUBMIT FORM Button.

You will be re-directed to PayPal site where you have the option to pay using PayPal OR (scroll to the bottom of the page) and you will see a Pay by Debit or Credit Card option - where you can check out as a PayPal Guest without having to have a PayPal account.  

NEW MEMBERS - Welcome to USSPWDC!  

USSPWD Club currently offers Four Membership Types.  

• Individual:  Annual Dues $30.00.  Open to all persons 18 years of age and older and enjoys all USSPWDC privileges, including the right to vote and hold office. 
• Family:  Annual Dues $40.00.  Open to households including two adult members at least 18 years of age who are each granted one vote.  The two family members may each hold office, vote and count for determination of quorum.  
• Junior: Annual Dues $5.00.  Open to persons 9-17 years of age and enjoy all USSPWDC privileges, except the right to vote and hold office and shall not count in the determination of a quorum.  This membership may automatically convert to a regular membership at age 18. 
• Qualified Complimentary Membership: New puppy owners members and those who adopt from PWDCA rescue will have their dues waived for the first year of membership provided that (1) they have never been members of USSPWDC Club and (2) if obtained from a breeder, the breeder of the prospective member's puppy is a current member of the USSPWD Club.

We are excited to welcome new puppies and adopted PWDCA rescue dogs, along with their owners to the USSPWDC.  Come join us and explore the wonderful world of PWD activities.  

Our new puppy owner and adopted rescue members must not have been members of USSPWD Club previously, and for puppies, the breeder must be a current Club member.  Dues will be waived for the remainder of the current membership year. 

 Note:  If you have adopted a PWDCA rescue dog, when filling out the information below please enter PWDCA as Breeder First Name and Rescue as Breeder Last Name!  This information is required.  

Hold Harmless Agreement:  With the submission of this membership application - I agree to hold harmless the USSPWD Club and its Board of Directors, members and the owners of the premises on which activities are held, from any claim of loss or injury which may be alleged to have been caused directly or indirectly to me or to my dog(s) while involved in any club activities.  I personally assume all responsibility and liability, and I enter the said premises, for whatever purpose, at my own risk.  

Consitituion and Bylaws: I (we) agree to abide by the constitution and Bylaws and Rules of both the Portuguese Water Dog Club of America (if a member) and the American Kennel club.  I (we) also agree to abide by the constitution and Bylaws of the United Sunshine State Portuguese Water Dog Club, a copy of which is posted at the following link:  Constitution and Bylaws
