Whereas, I as the parent/guardian hereby give my consent for my minor child to participate in the recreational programs sponsored by the Hollywood Police Athletic League and the Hollywood Brown Foundation. In those instances where it is necessary for my child to be transported to an athletic event or program sponsored by the Hollywood Police Athletic League. I give further consent for the transportation of my child in the Hollywood Police Athletic van to and from those particular locations.

In considerations of the opportunity for my minor child to participate in the events and recreational programs sponsored by the Hollywood Police Athletic League and transportation to and from particular programs and the events in the Police Athletic League van, I hereby agree on behalf of my minor child to release and hold harmless the City of Hollywood, its officers, agents and employees against any loss, damage or expense that may be suffered by the City of Hollywood from any claims, actions or suits for any personal injury or damage to property sustained by reason or as a result of my minor child’s participation in the recreational programs and events sponsored by the Hollywood Police Athletic League and the transportation to and from particular of the various recreational programs and events.

Nothing in this agreement shall be construed to affect in the way the Hollywood’s rights, privileges, and immunities as set forth in Section 768.28, Florida Statutes.

I further understand that if my minor child sustains an injury as a result of an accident while participating in a recreational program sponsored by the Hollywood Police Athletic league, I hereby consent for my child to be treated by any local licensed medical facility at the request or direction of a Hollywood Police Athletic League representative.

I hereby certify that I am the parent/guardian of the above referenced minor child.

I being the parent/legal guardian of the player named on the reverse side of this form, hereby consent to the use of photos, video, or audio recordings of myself or of my child, including (but not limited to) print, audio, video and web promotion. Such materials may be posted to Hollywood PAL and Hollywood Brown Foundation website or social media pages, or any other related websites, or used and reproduced in printed materials. I waive all claims to compensation or damages based on the use of my or my child's image, correspondence, or voice, and I also waive the right to inspect or approve the finished product. I understand that this consent is perpetual, that I may not revoke it, and that it is binding.

Parents/Spectators Code of Ethics & Agreement to Follow 

• I will encourage good sportsmanship by demonstrating positive support for all players, coaches, game officials, and administrators. at all times.

• I will place the emotional and physical well-being of all players ahead of any personal desire to win.

• I will support the coaches, officials, and administrators working with my child, in order to encourage a positive and enjoyable experience for all. 

• I will remember that the game is for the players, not for the adults. 

• I will ask my child to treat other players, coaches, game officials, administrators, and fans with respect.

• I will always be positive.

• I will always allow the coach to be the only coach.

• I will not get into arguments with the opposing team's parents, players, or coaches.

• I will not come onto the field for any reason during the game.

• I will not criticize game officials.

• Alcohol, illegal drugs, and unauthorized prescription drugs shall not be possessed, consumed, or distributed before, during, or after any game or at any other time at the field and/or game complex.

I agree to abide by and follow the Hollywood PAL program code of ethics. I also understand that my child, as well as all spectators coming to support my child, must also abide by and follow the code of ethics. I understand that any violation of the code of ethics may result in the expulsion of my child/children and myself from the Hollywood PAL. I also understand that should my child/children be expelled, that I am not entitled to nor will receive a refund from the Hollywood PAL or any other entity.

By my signature below, I am certifying that I have read and agreed to the above stipulations and agreements contained in this form. I bind myself, the minor, and all other assigns to the terms stated, and I certify that I have the legal capacity to do so.
