These sessions are designed for serious and dedicated pitchers looking to refine their mechanics, gain a deeper understanding of their motion, and make long-term progress in their pitching development.

Because these sessions require a high level of committement and focused time, athletes must apply to be considered. Not all applicants will be accepted, as availability for these sessions is extremely limited. 

Note: Pitchers under the age of 11 are usually not accepted, due to physical and skill requirements. But you may still apply if you feel there are special circumstances.

Optional: There is a section in this form to upload video clips for Coach Harley to review. These are not mandatory now, but submitting them with your application will remove a step later and help Coach Harley review your application faster.

• Front Angle: From the catcher's view (not too far away)
• Back Angle: From CF/2B view (directly behind pitcher)
• Side Angle: Clear chest view of the pitcher (not her back)
• Please keep videos short - one pitch per angle is perfect!

TERMS & CONDITIONS

Application Review & Selection Process

Submitting an application does not guarantee approval. Coach Harley will personally review all applications and select the athletes who are the best fit for this intensive training. Only serious and committed pitchers will be considered.

Scheduling Acknowledgement

I understand that Intensive Training Sessions are currently only offered on Saturdays, with the possibility of additional days being added in the future. I also understand that Intensive training sessions are a minimum of 2 hours.

Catcher Policy

I understand that Coach Harley will not provide a catcher for this intensive session. If you would like your athlete to throw to a catcher, you will have to provide one. However, most of the session will focus on drills where a catcher is not necessary. If you cannot find a catcher, your athlete will throw into a pitching target instead.

Payment Policy

I understand that the cost of an Intensive Training Session is $150 per hour, with a 50% deposit required to reserve your session. The remaining balance is due at the conclusion of the session.

Cancellation Policy

If you need to cancel more than 5 days in advance, you may choose to receive a refund of your deposit or reschedule your training session with no penalty. If you cancel within 5 days of your scheduled session, your deposit is non-refundable.

Minor Filming and Social Media Content Use Waiver

By checking this box, I, the parent listed on this form, hereby give consent for my child, minor listed on this form, to be filmed and photographed during their participation in activities organized by Diamond Elite Academy & Harley Rose Pitching. I understand that these recordings may be used by Diamond Elite Academy & Harley Rose Pitching for promotional purposes, including but not limited to social media posts and other marketing materials.

I grant Diamond Elite Academy & Harley Rose Pitching permission to use, reproduce, distribute, and display any recordings or photographs of my child taken during their participation in Diamond Elite Academy & Harley Rose Pitching activities, without compensation or approval rights, on Diamond Elite Academy's & Harley Rose Pitching's official social media channels and other promotional materials. I understand that this permission is granted for an indefinite period unless revoked in writing.

I understand that Diamond Elite Academy & Harley Rose Pitching will take reasonable measures to protect my child's privacy and personal information in accordance with applicable laws and regulations.

I have read this waiver carefully, fully understand its contents, and by checking this box, I voluntarily agree to its terms.

RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT ("AGREEMENT") DIAMOND ELITE ACADEMY

I request my child (identified on this form) be permitted to participate in the identified sport/athletic Diamond Elite Academy clinic activity and agree to the following: I understand and agree that my child’s participation in this activity may expose him/her to risks of serious personal injury. I will counsel my child so he/she understands that it is important for his/her safety and the safety of others, to follow all instructions of the Diamond Elite Academy coaches and staff.

I agree that I am responsible for my child’s conduct while he/she is at camp. In consideration for my child’s participation in this activity, on behalf of myself and my child, I release, discharge and hold harmless Diamond Elite Academy, its coaches, volunteers, employees, and owners from all liability, claims, costs, and expense, arising out of these activities which may result in injury or illness to my child. Diamond Elite Academy is not responsible for, and will not provide any medical, dental, hospital, or laboratory fees due to injury while participating in any clinics held by Diamond Elite Academy. I am the parent/legal guardian of the child.

I further agree that Diamond Elite Academy staff and owners are authorized to obtain and authorize emergency medical treatment for my child, up to and including emergency hospitalization and surgery. I agree to be personally responsible for any related medical expenses. On behalf of my child, and myself, I further release Diamond Elite Academy and any medical provider of emergency treatment to my child for any related liability.
