Applicant Acknowledgment and Authorization
If this application leads to employment, I understand that any false or misleading information provided in my application or interview may result in termination of employment. Neither the acceptance of this application nor subsequent employment, regardless of position or duration, creates a contract or guarantee of continued employment. I understand that all employment with Mountain Dogs Pet Resort (“Mountain Dogs”) is at will, meaning either the employee or the company may terminate the employment relationship at any time, with or without cause or notice, unless otherwise required by law. No policy, handbook, or communication shall alter this relationship unless a written agreement is signed by the Owner or General Manager. I understand that Mountain Dogs reserves the right to revise, modify, or eliminate policies, benefits, and procedures at its discretion.
I agree to provide a two (2) week notice prior to my last day of employment.
I authorize Mountain Dogs to investigate all statements contained in this application and to contact schools, former employers (unless otherwise noted), and references. I release all parties from liability for any information shared during such inquiries.
I acknowledge that Mountain Dogs maintains a drug and alcohol policy, which may include pre-employment and post-employment testing, and that compliance with this policy is a condition of employment. Employment for certain positions may be contingent upon meeting the physical requirements essential to safely perform the duties of the job.
I understand that, as part of the application process, Mountain Dogs may request an investigative consumer report from a consumer reporting agency, which may include information regarding my credit history, character, general reputation, personal characteristics, and mode of living. Upon written request, Mountain Dogs will provide additional details about the nature and scope of such report, as required by the Fair Credit Reporting Act (FCRA).
