2026 ACHE-WI Board Application

Submit your application to be considered for service on the 2026 Board of Directors
Complete the application below. Include all required information. After submitting your application, you will receive a message that includes a link to a form that you can share with your supporters or you can use this link. A minimum of 5 supporters are required for applications to be considered complete.

Complete applications are due by Monday, November 3. All candidates will be notified of the election results after voting closes.

Save the date: candidates who are elected to serve on the board will be invited to the board meeting on November 21, 2025 from 8:00 am - 12:00 pm in Madison.
