  • As a client of Impress Tax Services, we need to collect the following information from you to begin to prepare your income tax return. 

    Important: We require one form per person per tax year for whom we are preparing taxes.  The taxpayer must submit their own information, as there are places where they must digitally sign.

  • Tax Credits Due Diligence Questionnaire

    Due Diligence refers to the thoroughness and care that our Tax Professionals have to gather, verify, and analyze from the client to accurately calculate tax liabilities, etc.
  • You are being provided with this questionnaire because you have previously claimed one of these credits or indicated that you will claim one this year. The questionnaire is designed to assist us in qualifying you for some valuable refundable tax credits.

    We are required to ask under the Due Diligence Requirements of the new Tax Laws. Please complete them in as much detail as possible. Not all questions will apply to all taxpayers. If they do not apply to you, please answer them as 'NA' (not applicable) so that we know you have read the question and didn't just miss it.
    Abbreviations used on the Questionnaire:

    CTC: Child Tax Credit
    ACTC: Additional Child Tax Credit
    EITC: Earned Income Tax Credit (Low Income Households)
    AOTC: American Opportunity Tax Credit (College Credit)

    FOR NEW CLIENTS with Dependents: Please remember to submit copies of social security cards, birth certificates, and proof of residency (possibly school records or medical records showing the dependent's name and YOUR address).

    FOR TAXPAYERS CLAIMING COLLEGE TAX CREDITS: Please remember to submit copies of the Form 1098T that you received from the college AND a copy of the Bursar's statement from the college showing the Spring and/or Fall semester charges and payments. The Credit applies to all payments made in cash or via student loans to tuition, fees, and related expenses. IT does NOT apply to Room and Board or travel expenses.

    FOR TAXPAYERS CLAIMING HEAD OF HOUSEHOLD FILING STATUS: It is important to complete both the dependent questions and the Household Expense worksheet on page 3. If you are divorced or legally separated, please remember to include a copy of the divorce decree and/or separation agreement. [NOTE: If the divorce is NOT final or you and your spouse lived together AT ANY TIME in the last 6 months of the tax year, you CANNOT claim Head of Household filing status].

  • Tax Preparation Checklist 

    The documents you need for tax prep depend on your situation. To help you prepare for filing your taxes with Impress Tax Services, we’ve created a checklist to help you know what forms you'll need.

    Personal Information

    Tax Identification Numbers are mandatory items on your checklist. All taxpayers will need the following to do their taxes.

    • Your social security number or tax ID number
    • Your spouse’s full name, social security number or tax ID number, and date of birth
    • Identity Protection PIN, if one has been issued to you, your spouse, or your dependent by the IRS
    • Routing and account numbers to receive your refund by direct deposit, or pay your balance due if you choose
    • Foreign reporting and residency information - if applicable

    Dependent(s) Information

    Parents and caregivers should gather this information as they review what they need to file their taxes.

    • Dates of birth and social security numbers or tax ID numbers
    • Childcare records (including the provider's tax ID number) if applicable
    • Income of dependents and of other adults in your home
    • Form 8332 showing that the child’s custodial parent is releasing their right to claim a child to you, the noncustodial parent (if applicable)

    Sources of Income

    Many of these forms won’t be needed to file taxes every year. For example, you will only receive the investment forms you may need to file your taxes if you have had distributions or other activity.

    • Records of all expenses — check registers or credit card statements, and receipts

    • Office in-home information, if applicable
    • Record of estimated tax payments made (Form 1040–ES)

    Rental Income

    • Records of income and expenses
    • Rental asset information (cost, date placed in service, etc.) for depreciation
    • Record of estimated tax payments made (Form 1040–ES)

    Retirement Income

    • Pension/IRA/annuity income (1099-R)
    • Traditional IRA basis (i.e., amounts you contributed to the IRA that were already taxed)
    • Social security/RRB income: SSA-1099, RRB-1099

    Savings & Investments or Dividends

    • Interest, dividend income (1099-INT, 1099-OID, 1099-DIV)
    • Income from sales of stock or other property (1099-B, 1099-S)
    • Dates of acquisition and records of your cost or other basis in property you sold (if basis is not reported on 1099-B)
    • Health Savings Account and long-term care reimbursements (1099-SA or 1099-LTC)
    • Expenses related to your investments
    • Record of estimated tax payments made (Form 1040–ES)
    • Transactions involving cryptocurrency (Virtual currency)

    Other Income & Losses

    • Payment Card and Third Party Network Transactions - 1099-K
    • Gambling income (W-2G or records showing income, as well as expense records)
    • Jury duty records
    • Hobby income and expenses
    • Prizes and awards
    • Trust income
    • Royalty Income 1099–MISC
    • Any other 1099s received
    • Record of alimony paid/received with ex-spouse’s name and SSN
    • State tax refund
  • Tax Preparer Agreement

  • Services to be Provided

    The Tax Preparer agrees to provide the following services to the Client:

    Preparation and filing of federal income tax returns.
    Preparation and filing of state income tax returns (if applicable).
    Review of the Client's financial documents and tax records.
    Providing tax planning advice and guidance as needed.

    Fees and Payment

    1. The Client agrees to pay the Tax Preparer a fee for the services provided.
    2. Payment shall be made in full upon completion and delivery of the completed tax returns.
    3. The Client understands that additional fees may apply for services beyond the scope outlined in this agreement, and will be discussed and agreed upon in advance.
    4. If payment for tax preparation services is chosen to be deducted from your tax refund through a third-party bank or processing company, please note: In the event your refund is delayed, offset, rejected, or not issued by the IRS or state agency for any reason, you are still fully responsible for paying the tax preparation and refund processing fees directly to Impress Tax Service.
    5. Failure to make payment within the agreed timeframe will be considered negligence, and your account may be sent to automatic collections for the outstanding balance.

    Responsibilities of the Parties

    1. The Tax Preparer agrees to exercise due diligence and care in preparing the Client's tax returns in accordance with applicable tax laws and regulations.
    2. The Client agrees to provide all necessary information and documents to the Tax Preparer in a timely manner for the preparation of the tax returns.
    3. The Client acknowledges that they are ultimately responsible for the accuracy and completeness of the information provided to the Tax Preparer.

    Confidentiality

    Both parties agree to maintain the confidentiality of any sensitive information exchanged during the course of the engagement.

    Term of Agreement

    This agreement shall remain in effect for the duration of the tax preparation engagement and shall end upon completion of the services.

    Termination

    Either party may terminate this agreement with written notice to the other party. In the event of termination, the Client agrees to pay for any services rendered up to the date of termination.

    Governing Law

    This agreement shall be governed by and construed in accordance with the laws of the state of the Client and Tax Preparer.

  • Authorizing a representative

    When you authorize a representative you are letting that person represent you for income tax matters at the level of authorization you specify, for the tax year or years you indicate. Income tax matters include issues related to information on your tax return.
  •  Your responsibilities

    It is your responsibility to monitor and understand the transactions your representative completes on your behalf, and to make sure that the information for your representative is current.  It is your responsibility to let us know immediately should your marital situation change to separated or divorced, or should you decide to retract the authorization.

I understand that false information may be grounds for the federal government to seek criminal charges against me. I also release IMPRESS TAX SERVICES from any liability related to the information provided. My signature also authorized IMPRESS TAX SERVICES to file my taxes.

    The Taxpayer has provided all answers to the above questions to be true and correct to the best of the taxpayer's knowledge.

    Taxpayer has been informed that claiming a dependent for EITC/CTC/AOTC//HOH or other can result in an audit, fines, and penalties if information provided to the preparer is incorrect.
