Recognising heroes who are empowering communities.
North West London Integrated Care System (ICS) health equity programme is seeking nominees for awards for the category of: Recognising north west London heroes who are empowering communities.

If you would like to nominate person or organisation from NW London's voluntary sector, NHS or local authority who has contributed to reducing health inequalities in their community, please complete your nomination by 12 noon Friday 31 October 2025.

Please make sure you complete the form in full, to give your hero the best chance with their nomination.

We hope to recognise new individuals and organisations who have not won previously, but that does not mean teams who have won before can't be nominated again.

The winners will be announced at the ICS Health Equity Summit on 25 November 2025.
