  • Erosion Prevention and Sediment Control Permit Notice of Termination Form

  • General Information:

  • Format: (000) 000-0000.
  • NOTICE OF TERMINATION (NOT) REQUEST INFORMATION

  • What You Need to Do:

    • Remove debris from impervious surfaces, catch basins, BMPs, etc.
    • Provide City with as-built drawings for all permanent BMPs.
    • If any of the permitted area is changing ownership, an Erosion Prevention and Sediment Control Permit Transfer Form is required to be submitted to the City. *NOTE* each parcel of record (building site) must have its own permit.
  • What You Need To Know:

    • Your site will be inspected by the City to ensure compliance.
    • You will receive a confirmation of the NOT being accepted; unless notified otherwise, the NOT automatically becomes effective 15 days after the received/postmarked date.
    • A completed NOT does not qualify for termination of any permits other than the City’s EPSCP.
    • A completed NOT does not exempt any long-term BMP maintenance plans.
  • Acknowledgments and Agreements:

  • By signing below, the applicant certifies under penalty of law that all stormwater discharges associated with construction activities previously authorized at the identified site have been eliminated or that the applicant is no longer the permittee for this site. Final stabilization means that all soil-disturbing activities at the site have been completed, no rills or gullies are present on site, all temporary BMPs (e.g., silt fence, silt basin) have been removed, and a uniform perennial vegetative cover with a density of at least 70% covers all of the disturbed site, or equivalent stabilization measures have been placed.
It is understood that by submitting this Notice of Termination, the permittee is no longer authorized to discharge stormwater associated with construction activities under the Erosion Prevention and Sediment Control Permit. Discharging pollutants in stormwater to waters of the United States without authorization from a National Pollutant Discharge Elimination System (NPDES) permit is unlawful under the Clean Water Act.
Furthermore, submission of this Notice of Termination does not release the permittee from liability for any violations of the previously issued permit, Chapter 54: Stormwater and its subsections of the City of North Mankato, or the Clean Water Act.
