1. Program Participation
For my child to participate in the Bellevue Academy of Arts & Music programs at the following locations:
- Bellevue Academy of Arts & Music, 12356 Northup Way, Suite 111, Bellevue WA 98005, and its buildings and all businesses associated with the North Park Business Center at 12328–12372 Northup Way, Bellevue WA 98005
- Northwest Pianos, 13310 Bel-red Road, Suite 100, Bellevue WA 98005, and its building including the MOX Boarding House at 13310 Bel-red Road, Bellevue WA 98005
I hereby waive, release, and discharge any and all claims for damages for personal injury or property damage that may occur as a result of participation in said programs.
This release is intended to discharge in advance the venues of Bellevue Academy of Arts & Music, Northwest Pianos, MOX Boarding House, Resonance at Soma Towers, teachers, employees, family members, volunteers, and peers (students) from liability, even if such liability arises from perceived negligence. It is understood that some activities involve an element of risk, and knowing those risks, I hereby assume them. This waiver, release, and assumption of risk is binding on my heirs and assignees.
2. Conduct and Withdrawal
It is understood that my child must abide by the rules and codes of conduct at all program venues. If the child exhibits excessive aggression or disruptive behavior, program staff reserve the right to dismiss the child without refund.
Important Disclosure:
Students may be withdrawn from the program for any of the following:
- Unresponsiveness to instruction: Demonstrating one or fewer active responses out of five continuous prompts by the end of the second class, while over 90% of other students follow prompts. Examples include “Please bring your book and sit” or “Open the book to page 15.”
- Disruptive behavior: Significantly impeding class (pauses longer than 30 seconds per occurrence, up to three times per class, due to verbal, visual, or physical actions, such as screaming, pounding instruments, running, talking over the instructor, throwing items, or repeated behaviors). Parents of students with learning disabilities should consult staff to develop the best approach.
- Inability to follow instructions or performing the opposite action when prompts are imperative to the curriculum.
Program staff reserve the right to terminate enrollment for misconduct, including but not limited to:
- Physical contact or abuse toward others
- Verbal abuse or use of foul language
- Threats or intimidation
- Tampering with or damaging another’s property
- Unsanitary behavior
- Use, sharing, or coercing others to use inappropriate substances
All incidents will be documented in a detailed case report.
3. Medical Consent
If the child is a minor, the undersigned parent or guardian authorizes Bellevue Academy of Arts & Music to provide emergency medical treatment as deemed necessary while participating in program activities at the listed venues.
4. Consent to Share Work as Data
By participating in BAAM programs, which include MathPiano®, PianoTowers™, and other research programs, you agree that all work created as part of participation—including recordings, compositions, assessments, and other materials—may be collected and used by BAAM for educational, research, and program development purposes.
- Collected data may be analyzed to:
- Assess student progress
- Improve teaching methodologies
- Develop new learning tools
BAAM may use aggregated, anonymized data for research studies, presentations, publications, and other academic or educational activities. Personal identifying information, including full name, address, or other sensitive details, will not be shared. Limited information, such as first name, age, and assessment results, may be publicly disclosed solely to demonstrate program outcomes and student achievements.
5. Use and Return of Materials
For students enrolled in general music programs, all MathPiano® and PianoTowers™ materials are copyrighted original works and must remain at BAAM. These materials, whether physical or digital, may not be taken home, retained, copied, or distributed.
Upon termination of enrollment, all materials must remain with BAAM. Unauthorized possession or duplication of MathPiano® or PianoTowers™ materials may result in legal action or other remedies deemed necessary by BAAM.
