Glenview Environmental Restoration Network (GERN) Volunteer Agreement & Waiver

VOLUNTEER INFORMATION:

1. Assumption of Risk & Liability Waiver
I understand that participation in Glenview Environmental Restoration Network (“GERN”) volunteer activities may include physical activity, outdoor labor, and exposure to natural or man-made hazards. These activities may include, but are not limited to, lifting, bending, working with tools, exposure to insects, plants, uneven terrain, and other potential risks of injury.

By signing below, I voluntarily assume all risks of injury, illness, or damage to person or property that may arise from participation in any GERN activity. I agree to release, waive, and hold harmless GERN, its organizers, sponsors, volunteers, and partners from any and all claims, liabilities, damages, or expenses that may result from my participation.

2. Parental Consent (for Volunteers Under 18)
I, the undersigned parent or legal guardian, give permission for my child named above to volunteer with GERN. I have read and understood this waiver and consent to my child’s participation. I accept full responsibility for any injury or loss that may occur to my child while volunteering and agree to hold harmless GERN and its representatives from any claims or damages arising from participation.

3. Medical Authorization
In the event of illness or injury while participating in GERN activities, I authorize GERN staff or volunteers to obtain necessary emergency medical treatment on behalf of myself (or my minor child). I understand that I am solely responsible for any resulting medical expenses.

4. Photo & Media Release
I grant GERN permission to take photographs and videos during events and volunteer activities. I authorize GERN to use these images for promotional, educational, and marketing purposes, including social media, websites, and printed materials, without compensation.

5. Privacy & Data Consent
I understand that any personal information provided will be used solely for volunteer coordination, communication, and safety purposes, and will not be sold or shared outside GERN.

6. Acknowledgment and Signature
By signing below, I acknowledge that I have read, understood, and voluntarily agree to this entire Volunteer Agreement & Waiver. I understand that this document affects my legal rights and that I may wish to consult with an attorney before signing.
