Photo permission agreement

By submitting this form, you confirm that you have the right to share the provided information and images/videos and grant Surfers Against Sewage permission to use them in our online and print communications, including but not limited to, website, social media, reports, paid advertising, and promotional materials. I understand that these photos/videos/stories may be used for the purpose of promoting SAS and its activities. I also understand that once my consent is given, it will remain in effect unless I revoke it in writing. I waive any right to inspect or approve the finished photographs or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown.

* Before uploading your photos *

1. Rename your photo files

Use this format: MMC_SURNAME_01
Replace SURNAME with your surname and number each photo in order (01, 02, 03, etc.).
Example: MMC_SMITH_01

2. Check who appears in your photos

If any child under 18 or vulnerable adult is identifiable in your photos, including through a school, nursery or group logo, written permission must be obtained before we can use those images.

3. Upload the required permission forms

Each identifiable person must have their own completed Child and Vulnerable Adults Permission Form, signed directly by their parent or guardian.

Permission signed by a school, nursery, Scout group or other organisation on behalf of a child is not sufficient.

Upload one completed permission form for every identifiable person who requires permission. If you don't have all the required forms, don't submit photos containing those individuals.
Do any of the images you're uploading contain identifiable children under 18 or vulnerable adults?
How many children under 18 or vulnerable adults are included in the photos you are submitting?
Upload a completed Child and Vulnerable Adults Permission Form for each identifiable person requiring permission.

Matching individuals to their permission form

For each photo you upload, you must provide the full name of every identifiable child under 18 or vulnerable adult shown, and their location within the photo.

List the filename first, followed by each person’s full name and position in the photo.

Examples:
- MMC_SMITH_01 – Jane Smith (centre), John Smith (second from the left)
- MMC_SMITH_02 – Jane Smith (left), John Smith (right)
