📄 Program Commitment Agreement
By registering, I understand and agree that I am committing to a minimum of 3 months of participation in the selected class(es).
Tuition is due monthly and is non-refundable, regardless of attendance or early withdrawal.
A valid Credit/Debit Card must be on file. Your account will be drafted on the on the 1st of every month. If payment is unable to be drafted a $30 late fee will be applied.
⚠️ Liability Waiver
I acknowledge that participation in physical activities involves an inherent risk of injury. I release and hold harmless the program directors, instructors, volunteers, and facility owners from any and all liability, claims, or demands arising out of or related to any loss, damage, or injury that may occur while participating in or attending classes, events, or practices.
📸 Media Release
I grant permission for photos and/or videos of myself or my child taken during classes, practices, or events to be used for promotional purposes including, but not limited to, social media, websites, and print materials. I understand no compensation will be provided for the use of these images.
