Fill out the form carefully for registration. Your DOB must be no later than JUNE 01, 2011 to enroll in this course. DO NOT COMPLETE THIS FORM IF YOUR DOB IS AFTER JUNE 01, 2011, YOU WILL NOT BE ADMITTED TO CLASS. This class runs Monday-Friday after school each day from 3pm-6pm in the media center.
