This form is for fiscally sponsored projects under CFA to request additional services beyond the basic services we provide.

Included at no extra cost:

• Standard Donorbox setup

• Basic fiscal sponsorship template and design

• Two (2) full webpage passes for photo/video changes, supplementary uploads, or replacements (These should be uploaded as full documents with all notes in one document to be passed on to our web designer)

Activate automatic receipts sent by Donorbox to your email address to notify you each time you receive a donation via Donorbox. Campaigns without this option are able to view donations through balance request forms found on the member portal.

Crowdfunding Donorbox Campaign Setup

Full setup and configuration of a dedicated crowdfunding campaign within Donorbox, including campaign duplication to separate crowdfunding funds from the primary donation page for clear tracking and reporting. This service includes enabling crowdfunding features, uploading additional campaign images, adding and optimizing campaign copy (title, description, and supporting text), and ensuring the project’s account is properly connected to Donorbox with email notifications enabled.
