CFA Service Request: Notarization / Certified Copies
Cinematography for Actors Institute (CFA) provides notarization and certified copy services for our fiscally sponsored projects, as well as general support needs for our filmmakers. Please submit this request form so we can review, track, and process your request.

Service Options & Fees:

Option A: Certified CFA copies / CFA notarized documents: $65 minimum (includes mailing fees if required). For CFA fiscally sponsored projects only.

Option B: General notarization (in-person only): $15 per signature + $65 convenience fee (minimum).

Must be completed at CFA Studio in Los Feliz, CA. Bring original documents and two forms of ID (including one government-issued photo ID).

Additional charges may apply for multiple signatures, certified copies, or mailing.

Once submitted, a request ticket will be generated, and a CFA team member will follow up with next steps within 3-5 business days.
