FINE PRINT - Please read, your signature confirms agreement to the following:

1) MINORS Persons under the age of 18 must be accompanied by a legal guardian or staff at all times. The parent/guardian must sign this application.

2) HOLD HARMLESS I understand that the Wabash County Animal Shelter will be held harmless should any animal that I am working with or come into contact with causes any property damage and/or injuries. Any damages to my property and/or injuries will be my reponsibility.

3) SMOKE/VAPE Wabash County Animal Shelter is a smoke/vape free property. IF you feel the need, you must sign out, leave the property, and dispose of any trash generated properly. This is your 1st and ONLY warning. Failure to comply will result in immediated revoking of your volunteer / or community hours privileges.

4) CONFIDENTIALITY Any information you may see or hear pertaining to the Wabash County Animals Shelter, including but not limited to law enforcement issues, records, cases, and employee issues, adoptions, surrenders, euthanasia, names, addresses, or any other private information is confidential. This information may not be discussed with anyone aside from the appropriate staff members, or board members. Staff and volunteers are prohibited from discussing any Wabash Animal Shelter issue with representatives of the media, or on social media, without prior WRITTEN permission. If I am approached by any media or concerned citizen I will refer them to, and contact immediatly, the Animal Shelter Manager or Board President. IF a staff member or volunteer fails to follow this agreement, their position with the Wabash County Animal Shelter will be terminated immediately.

I understand the importance of each of these conditions and will abid by them. IF I have any questions about any of the above condidtions, I will contact the Animal Shelter manager. This application is dated as of the date of its submission.
