Workshop Terms and Consent
Upon signing this form, I acknowledge and agree to the following terms:
1. My child will be supervised by All In Arts staff during the workshop and will participate in festive crafts and activities.
2. I have informed staff of any medical conditions, allergies, or special needs, and consent to appropriate care being provided.
3. I give permission for All In Arts staff to contact me or seek medical treatment in case of emergency.
4. I understand that my child is expected to follow staff instructions and behave safely; disruptive behavior may result in dismissal.
5. I accept that All In Arts, its staff, and volunteers are not responsible for personal injury, loss, or damage to belongings.
6. By booking this special offer, I agree to pay the deposit and understand it's non refundable and to promptly pay the remaining balance when invoiced in January.
7. I understand if I need to cancel my booking for illness my booking can be used on another date but my deposit is non refundable.
8. I agree to drop off and collect my child at the allocated workshop times.
9. By signing this form I confirm that I have read, understood, and agree to these terms.
