In enrolling at Limitless Hockey participant understands that attending the programs and using Limitless and the facilities does so at his/her own risk. Limitless; owners, employees, or agents shall not be liable for any damage whatsoever arising from any personal injury or property loss sustained
by participant with his/her family in or about any programs on the premises. Participants and parents assume full responsibility for all injuries and damages which occur in or about any programs on the premises, He/She/They does hereby fully and forever release discharged hold harmless Limitless all
associated facilities and its owner, employees, and agents from any and all claims, demands, damages or rights of action, present or future resulting from any person’s participation in any programs or use of the facility. In addition, he/she/they agree(s) to follow the rules of conduct and
play, set by Limitless. Failure to do so may result in suspension from participation.
Consent:
I the undersigned parent or guardian/participant do hereby grant authority to the staff at Limitless to render a judgement concerning medical assistance or hospital care in the event of an accident or illness during my absence.
Privacy Policy:
It is the policy of Limitless that personal information, such as your name, personal postal and email address, or personal telephone number is private and confidential. Because your privacy is important to us, Limitless maintains privacy policies to protect your personal information.
Personal Information:
Personal information, such as your name, personal postal and email
address, or other personal information is collected only when you voluntarily provide it. Such information is received when you register. The personal information you provide to us is forwarded only to the person or department equipped to handle your request and is used only to respond to
your inquiry. Personal information will not be sold to any third parties.
Photo & Video Policy:
I do hereby authorize Limitless and its assigns to utilize any and all
photographs, pictures or other likeness of me or anyone assigned guardianship to me, as they deem appropriate in its promotional materials or team films in any of the following:
1. Website 2. Social Media 3. Print Advertising 4. Marketing Material
I hereby affirm that such a release does not constitute any form of compensation, including royalties arising from the photographs/videos to my benefit. I understand and agree that photographs in the possession of the Limitless Hockey (Ashley Meier) shall become the property of Limitless. I hereby waive my right to inspect or approve the photographs by which my likeness appears. I hereby hold harmless, release, and forever discharge Limitless (Ashley Meier) from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons
acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
ANTI-BULLYING POLICY
*Please review this policy with your child before attending camp.
Limitless (Ashley Meier) will not tolerate any bullying during the camp whether on or off the ice. We are here to support player development, and no player shall be in fear or worry during the camp. Any bullying MUST be reported immediately to a Group Leader or Instructor. Limitless reserves the right
to immediately remove the accused from the camp if deemed necessary. No refund will be provided under this circumstance. This includes name calling, hitting, punching, kicking, shoving, etc., threats to any player.
Drop Off / Supervision:
Please make sure that you are on time. Our schedules are jam-packed with
events, so it is important that we start promptly. Also ensure you are on time for check-in. Players are not permitted to wander on their own AT ANY POINT during the camp. If you need to go anywhere you MUST let your Group Leader know beforehand and get his/her approval.
Player Discipline:
During our programming we cover a lot of content, and therefore it is very
important that we have players attention throughout our sessions. Any player(s) not following the behavioural protocols will receive a STRIKE. Any player receiving 3 strikes over the course of the week will be removed from their current activity and group for the remainder of the day. At the conclusion
of the day, a Limitless team member will be in contact with the player’s parent to address the issue and answer any questions or concerns at that time.
We Look For 3 Keys In Players:
1.Eyes on the coach when they are addressing the group.
2. No Double Talking - Talking when the coach or other players are speaking to the group.
