Volunteer Safety & Media Release Waiver

Serving our community with care, dignity, and compassion

The Lifekind Foundation Food Pantry is committed to supporting individuals and families in need by providing nutritious food and essential resources. We strive to serve our community with compassion, dignity, and respect, creating a welcoming environment for all.

Volunteers must be at least 16 years of age unless accompanied by a parent or guardian.

Volunteer Waiver and Release of Liability
By signing below, I acknowledge and agree to the following terms and conditions of my volunteer service with the Lifekind Foundation:

Assumption of Risk
I understand that volunteering may involve physical activity, lifting up to 30 pounds, use of equipment, and interaction with members of the public. I voluntarily assume all risks associated with my participation, including but not limited to injury, illness (including communicable diseases such as COVID-19), or property damage.
Release of Liability

I hereby release, waive, and hold harmless the Lifekind Foundation and its affiliated entities, officers, directors, employees, agents, and volunteers from any and all claims, demands, causes of action, liabilities, or damages arising out of or in connection with my volunteer activities, including any acts of ordinary negligence, to the fullest extent permitted by law.
Medical Treatment

I understand that the Lifekind Foundation does not provide medical or health insurance coverage for volunteers. I am solely responsible for any medical expenses or treatment costs arising from my participation.

Compliance with Policies
I agree to follow all safety guidelines, instructions, and policies provided by the Lifekind Foundation during my volunteer service. I understand that failure to follow safety instructions may result in termination of my volunteer service.

Media Release
I grant permission to the Lifekind Foundation to use photographs, video, or audio recordings taken of me during volunteer activities for promotional, educational, or fundraising purposes in any media now known or later developed, including print, online, and social media platforms. I understand that I will not receive compensation and that my image will be used responsibly and consistent with the Foundation’s mission. I may revoke this consent in writing at any time, but revocation will not apply to materials already produced or published.
 
I have read and understood this waiver and release. I am signing it voluntarily and understand that by doing so, I am waiving certain legal rights.
