    This form replaces the previose Word document used for submission of the CCCBR affiliation information.

    This form will ask for the following information

    * Your societies name and type

    * Your contact details

    * Your societies membership numbers

    * Details of how you wish to submit the payment

    * You representatives details

    * Your secretaries contact details

    * Your societies web page address

     

    It is recommended that you have this information to hand before filling out this form. Please work your way though it, and press submit at the end. A confirmation email, containing the submitted information, will be sent to you.

    Please note, following changes to the rules and standing orders of the Council at the 2026 Annual Council meeting, affiliation fees are due on 1st January 2027, and must be paid by 31st March 2027.
