If you are interested in applying for assistance with repairs to your home through the JD Sheth Neighborhood Revitalization Program, please complete the application below. All questions must be answered. Incomplete applications will not be considered.

-I acknowledge that this program is supported by grant funding that JD Sheth Foundation has received. The grants come with limitations on the amount of money spent per home and type of repairs that can be completed.

-I acknowledge that completion of this application does not guarantee acceptance into the JD Sheth Neighborhood Revitalization Program.

-I acknowledge that JD Sheth's coordination with contrators could take up to 90 days to get repairs scheduled and/or completed.
