Please describe the home environment. Do you live in an apartment, house with fenced in yard, etc.
Are there any other animals that live in the house? Other dogs or cats?
The Handler agrees to:

-Be at least 18 years old or accompanied by an approved adult.

-Follow all instructions provided by the Rescue regarding handling, feeding, exercise limits, and behavior.

-Keep the dog leashed at all times unless in a Rescue-approved fenced area.

-Never leave the dog unattended in a vehicle, home, or public space.

The Handler understands and participates at their own risk

The Rescue is not responsible for any personal injury, property damage, or financial loss arising from the outing

By signing below, the Handler acknowledges:

They have read, understand, and agree to this entire Agreement.
They assume full responsibility for the dog during the outing and will return dog by agreed date.
They will comply with all Rescue rules and instructions. FosterDocHouston retains ownership of dog unless adoption form is completed.
