To ensure we are able to process your return efficiently, we kindly request that all sections marked with an * are completed. Please ensure the following information is included for each individual failed implant:
● One completed form.
● The failed implant must be sterilized and placed in a sealed sterilisation pouch.
● Account number, postcode, or practice name (as per our records).
● Lot number (a sticker is preferred; if unavailable, a photocopy of the sticker is acceptable).
● Associated X-ray image.
● The treating clinician’s signature.

Please note that all failed implant data must be submitted to our supplier for approval before replacement implants can be issued. Once approved, we aim to dispatch replacements by the first Friday of the month. Should there be any issues or delays, a member of our team will contact you directly.
