    The Open Day is an event for all prospective students and their parents taking place on Saturday, November 22nd from 12:00 am - 4:30 pm in our academic building at Fraňa Kráľa 6 in Bratislava, where you can take part in the sample classes, meet our team, current students and their parents and experience the atmosphere of LEAF Academy.

    Registration via this form is mandatory for each visitor individually.

     

    ​Deň otvorených dverí je podujatie pre všetkých záujemcov o štúdium a ich rodičov, ktoré sa koná v sobotu 22. novembra v čase 12:00 - 16:30 hod v priestoroch LEAF Academy na Fraňa Kráľa 6 v Bratislave, kedy máte možnosť zúčastniť sa ukážkových vyučovacích hodín, spoznať niektorých našich súčasných študentov a ich rodičov, opýtať sa svoje otázky priamo členov tímu a zažiť atmosféru LEAF Academy.

    Registrácia je povinná pre každého návštevníka zvlášť.

  • Whereas, pursuant to Regulation (EU) 2016/679 of the European Parliament and of the EU Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, repealing Directive 95/46/EC (General Data Protection Regulation), also known as GDPR, we are obliged to verify that consent to the processing of the personal data, given by a child at the registration, has been granted or approved by the parent (or the person exercising parental responsibility) for children under 16 years of age.

  • I hereby give my express consent with the personal data processing by the group LEAF (hereinafter referred to as LEAF), with the managing organization LEAF with registered office at Krajinská cesta 32, 921 01 Piešťany, Slovakia, Organization ID: 42264723, recorded in the Civic Association Register maintained by the Ministry of Interior Affairs of the Slovak Republic under the number VVS/1-900/90-40088 for the purpose of:

    Consent to personal data processing for marketing purposes

    sending information about LEAF activities, invitations to events or events of third, cooperating parties and contacting with LEAF opportunities or other marketing activities of LEAF. I agree that LEAF, as the operator of this purpose, will process my personal data to the extent of name, surname, contact details, program/event I’m interested in. I give my consent for a period of 5 years from the date of my consent. I agree that LEAF should contact me via given contact details for this purpose and I request that such information, materials and offers be provided.

    Consent to the processing of images, audio and video recordings

    promotional purposes of LEAF with the production and use of images, audio and video recordings created for this purpose, through its publishing on the LEAF webpages, LEAF social media accounts and in any electronic and print materials promoting LEAF for the purpose of presenting the work and activities of LEAF to the public. I hereby give my consent to LEAF as the operator for this purpose with the processing of my personal data including my image, video or audio recording capturing myself, my name and surname. I give my consent for a period of 5 years from the date it was granted.

  • Na základe Nariadenia Európskeho parlamentu a Rady (EÚ) 2016/679 z 27. apríla 2016 o ochrane fyzických osôb pri spracúvaní osobných údajov a o voľnom pohybe takýchto údajov, ktorým sa zrušuje smernica 95/46/ES (všeobecné nariadenie o ochrane údajov) (ďalej ako „Nariadenie“) a zákona č. 18/2018 Z. z. o ochrane osobných údajov (ďalej ako „zákon č. 18/2018 Z. z.“) ak záujemca o registráciu na podujatie nedosiahol vek 16 rokov, je na spracúvanie jeho/jej osobných údajov potrebný súhlas jeho/jej zákonného zástupcu. Tomu bude zaslaný samostatný email na potvrdenie súhlasu.

  • Vyjadrujem svoj súhlas na spracúvanie mojich osobných údajov skupinou LEAF (ďalej LEAF), s riadiacou organizáciou LEAF so sídlom Krajinská cesta 32, 921 01 Piešťany, Slovenská republika, IČO: 42264723, zapísanému v evidencii občianskych združení vedenom Ministerstvom vnútra Slovenskej republiky pod číslom VVS/1-900/90-40088 za účelom:

    Súhlas so spracúvaním osobných údajov pre marketingové účely

    za účelom informovania o aktivitách LEAFu, zasielania pozvánok na podujatia alebo podujatia tretích spolupracujúcich strán a kontaktovania s ponukami príležitostí v rámci LEAFu, prípadne iných marketingový aktivít LEAFu. Súhlasím, aby LEAF ako prevádzkovateľ tohto účelu, spracúval moje osobné údaje v rozsahu meno, priezvisko, kontaktné údaje, program / event o ktorý mám záujem. Svoj súhlas udeľujem na obdobie 5 rokov od dňa udelenia súhlasu. Súhlasím, aby ma LEAF za týmto účelom kontaktoval na uvedené kontaktné údaje a zároveň žiadam o poskytovanie takýchto informácií, materiálov a ponúk.

    Súhlas so spracúvaním fotografií, audio a video záznamov

    za účelom propagácie a šírenia dobrého mena LEAFu s vyhotovením a využitím mojich fotografií, audio a video záznamov zhotovených pre tento účel, prostredníctvom ich zverejnenia na webových stránkach LEAFu, na účtoch LEAF na sociálnych sieťach a v elektronických a papierových materiáloch propagujúcich LEAF s cieľom prezentovania práce a činnosti LEAF verejnosti. Súhlasím, aby LEAF ako prevádzkovateľ tohto účelu, spracúval moje osobné údaje v rozsahu fotografie zachytávajúcej moju podobizeň, videozáznam alebo zvukový záznam, na ktorom som zachytený, moje meno a priezvisko. Svoj súhlas udeľujem na obdobie 5 rokov od dňa udelenia súhlasu.

    Zároveň beriem na vedomie, že za účelom mojej účasti na zvolenom podujatí organizovanom LEAFom a za účelom vyhodnotenia prínosu podujatia pre LEAF a účastníka dochádza k spracúvaniu mojich osobných údajov tak ako je uvedené v informácií o spôsobe spracúvania a ochrany mojich osobných údajov.
