BASEBALL 101 WAIVER

Acknowledgment and Assumption of Risk
I, the undersigned parent or legal guardian of the above-named child (“Participant”), acknowledge that participation in Baseball 101 involves physical activity, including running, catching, throwing, batting, and other related movements.

I understand that there are inherent risks of injury—minor or serious—that may occur as a result of participation, including but not limited to bruises, sprains, broken bones, or other physical injuries.

I willingly allow my child to participate in this program with full knowledge of these risks.

Waiver and Release of Liability
In consideration of my child’s participation in Baseball 101, I, on behalf of myself, my child, and our respective heirs, executors, and assigns, hereby release and hold harmless Baseball 101, its owners, coaches, volunteers, agents, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage sustained during or in connection with participation in Baseball 101 activities, whether caused by negligence or otherwise.

I further agree that Baseball 101 shall not be responsible for any medical costs incurred as a result of injury or illness sustained during participation.

Medical Treatment Authorization
In the event of an emergency where I cannot be reached, I authorize Baseball 101 coaches or representatives to obtain emergency medical care for my child as deemed necessary by qualified medical personnel. I understand that I am responsible for any medical costs incurred.
