Why We're Doing This Focus Group
We’d like to invite you to be part of a focus group. We’ll talk about how to make well checks for children, also known as a physical, easier for families with BadgerCare Plus insurance.
You are being invited because:
- You have BadgerCare Plus insurance
- You have 1 or more children under 5 years old
We are seeking a small group of 6–8 people to talk for about 60 minutes. We’ll ask for your thoughts and ideas about well visits. Your feedback will be used to improve our outreach efforts.
This form explains what the focus group is about so you can decide if you want to take part. Please read carefully, and if anything is confusing, you can ask questions. You can contact:

Your Privacy
Laws require GHC-SCW to keep your health information private.
- We will not use your name in reports or discussions about the focus group.
- The focus group will be audio recorded to help us create a report and presentation for an internal workgroup.
- We won’t ask for your name or other personal information.
- We ask that you also respect the privacy of what others say in the focus group.
Your Participation Is Voluntary
- Joining is your choice.
- You can skip any question or stop at any time.
- Saying yes or no will not affect your BadgerCare Plus insurance or healthcare.
Risks and Benefits
- There are no major risks. Some questions might feel a little personal.
- You can skip any questions or leave whenever you want.
- You won’t get medical help from this group, but your ideas could help improve care for others.
