Concierge Membership Terms & Conditions
By enrolling in a Concierge Membership with Fusco Care Services, you acknowledge and agree to the following terms and conditions:
Billing and Payments
Membership fees are billed automatically on a monthly basis, recurring on the same calendar date as the original enrollment date. Payment will be processed through the payment method provided at registration.
Cancellation Policy
Memberships may be cancelled at any time by submitting written notice via email to Info@fuscocare.com at least five (5) business days prior to the next scheduled billing date. Failure to provide sufficient notice may result in the next month’s charge being processed.
Refund Policy
Membership fees are non-refundable once a billing cycle has commenced. No prorated refunds will be issued for partial months or unused services.
Nature of Membership
The Concierge Membership is not an insurance plan, and it does not replace or constitute health insurance coverage. Membership fees cover enhanced access and convenience services as outlined within each tier. All routine medical care, laboratory testing, imaging, prescriptions, and other covered healthcare services will continue to be billed to your insurance or Medicare as appropriate.
Services and Limitations
Membership benefits are provided in accordance with the tier selected at enrollment. Certain services, such as additional IV drips, trigger point injections, or office visits beyond those included in the plan, may incur additional fees.
Telemedicine and in-person services are available during normal office hours, excluding holidays, and subject to provider availability.
Modification of Terms
Fusco Care Services reserves the right to amend membership pricing, included services, or these terms and conditions at any time. Members will receive a minimum of thirty (30) days’ written notice prior to the effective date of any material change. Continued participation in the membership program after such notice constitutes acceptance of the new terms.
Termination by Provider
Fusco Care Services reserves the right to terminate membership at any time for reasons including, but not limited to, non-payment, inappropriate conduct, or misuse of services.
