Cypress Golf Academy – Liability Waiver & Release
PLEASE READ CAREFULLY BEFORE PROCEEDING
This Liability Waiver and Release (“Waiver”) is executed by the undersigned parent or legal guardian (“Parent/Guardian”) on behalf of the participating minor child(ren) (“Participant”).
Assumption of Risk
I understand and acknowledge that participation in golf instruction, practice, training, camps, clinics, and related activities involves inherent risks that cannot be eliminated, including but not limited to:
• Physical injury resulting from golf clubs, golf balls, equipment, or physical activity
• Accidental contact with other participants, instructors, or spectators
• Weather-related conditions associated with outdoor activities
• Property damage, loss, or theft of personal belongings
• Transportation risks to and from the facility
I knowingly and voluntarily assume all such risks, whether foreseen or unforeseen.
Waiver and Release of Liability
In consideration for allowing my child(ren) to participate in programs operated by Westerdahl Golf LLC, doing business as Cypress Golf Academy at Sterling Country Club at Houston National, I agree as follows:
1. RELEASE AND WAIVER
I hereby release, waive, and discharge Westerdahl Golf LLC d/b/a Cypress Golf Academy, Sterling Golf Management, Sterling Country Club at Houston National, and all owners, officers, employees, instructors, coaches, agents, volunteers, and affiliates (collectively, the “Released Parties”) from any and all claims, demands, damages, losses, liabilities, or causes of action arising out of or related to my child(ren)’s participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.
2. MEDICAL AUTHORIZATION
I authorize the Released Parties to obtain emergency medical treatment for my child(ren) if deemed necessary. I understand and agree that I am financially responsible for any and all medical expenses incurred.
3. PHOTO AND VIDEO CONSENT
I grant permission for photographs, video recordings, and other media of my child(ren) to be used for instructional, marketing, promotional, and educational purposes without compensation.
4. COMPLIANCE WITH RULES
I agree that my child(ren) will abide by all program rules, safety guidelines, and instructions provided by staff. I understand that failure to comply may result in removal from the program without refund.
Acknowledgment and Agreement
By signing below, I acknowledge that:
• I am the parent or legal guardian of the Participant
• I have read and fully understand this Waiver
• I understand that this Waiver affects my legal rights
• I voluntarily agree to its terms on behalf of myself and my child(ren)
This Waiver is intended to be binding and enforceable under applicable state law.
